We, Members of the CSOC Youth Task Force, have come
up with therapy forms that we think will help make therapy sessions go a lot smoother. We first created these forms so that
youth would have a voice in choosing their therapist because as youth, we don't have any say-so in whom we would like our
therapist to be.
Our therapy forms consist of two pages. One page is for the youth to fill out before they are assigned
a therapist. This form was meant for youth to choose the type of therapist they would like, the ethnicity they would feel
comfortable with and where a good place would be to have sessions.
The second form also is for the youth. This form
is used before the session starts. The form is like an icebreaker for the patient. It asks youth whether they are hungry or
not and what type of mood they are in. This form was created to start out a friendly dialogue between the patient and the
therapist.
We, the CSOC Youth Task Force, believe that our forms will help better the system and make it more youth
friendly.

Angela Reyes and Inez Lovelead a youth-guided
care training for providers.
The Youth Development Team promotes youth-guided practices in the
children's service delivery systems. The team includes the CSOC Youth Coach and
the Youth Mentor. They oversee youth development activities across a variety of areas including education and outreach,
training, youth development, care coordination, and program policy and governance.
- The CSOC Youth Task Force,
a youth-led advocacy body that promotes youth friendly services, provide guidance to child serving systems about ways to make
services more youth-guided.
